SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding several forms and information gathering activities prior to their submission for clearance by the Office of Management and Budget (OMB), as well as for some forms and information gathering activities for which clearance has already been sought.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in a notice in the October 10, 2014, Federal Register (79 Fed. Reg. 61366). Comments may be mailed or faxed to the addresses and numbers listed in that notice.

Comments due by November 10, 2014

Comments are due by November 10, 2014, for the following forms, which have already been submitted to the OMB for approval: Statement of Funds You Provided to Another and Statement of Funds You Received (SSA-2854, SSA-2855).

Comments due by December 9, 2014

Comments are due by December 9, 2014, for the following forms and information gathering activities, which have not yet been submitted to the OMB for approval: Application for Supplemental Security Income (SSA-8000 (paper), MSSICS, MSSICS with Signature Proxy (attestation)); General Request for Social Security Records (eFOIA); and Incoming and Outgoing Intergovernmental Personnel Act Assignment Agreement (SSA-187 for incoming employees and SSA-188 for outgoing employees).
