SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding two information gathering activities prior to their submission for clearance by the Office of Management and Budget (OMB), as well as for some forms and information gathering activities that already have been submitted to the OMB for approval.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in a notice in the August 16, 2017, Federal Register (82 Fed. Reg. 38982). Comments may be mailed or faxed to the addresses and numbers listed in that notice.

Comments due by September 15, 2017

Comments are due by September 15, 2017, for the following items, which already have been submitted to the OMB for approval: Statement Regarding Date of Birth and Citizenship (SSA-702); Marital Relationship Questionnaire (SSA-4178, SSA-4178-Modernized SSI Claims System); and Medical Source Statement of Ability To Do Work Related Activities (Physical and Mental)(HA-1151, HA-1152).

Comments due by October 16, 2017

Comments are due by October 16, 2017, for the following items, which have not yet been submitted to the OMB for approval: Application to Collect a Fee for Payee Service (SSA-445, both private sector business and state/local government offices) and Redetermination of Eligibility for Help with Medicare Prescription Drug Plan Costs (SSA-1026-REDE, SSA-1026-REDE-Field Office Interview, SSA-1026-SCE, SSA-1026-SCE-Field Office Interview).
