The IRS has released the 2011 Form 8955-SSA and instructions. The form is substantially the same as the 2010 form. The IRS notes that it has created a page on its website about Form 8955-SSA, www.irs.gov/form8955ssa. Information about any future developments affecting the form will be posted on that page.
