SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding several forms and information gathering activities prior to their submission for clearance by the Office of Management and Budget (OMB), as well as for some forms and information gathering activities for which clearance has already been sought.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in a notice in the September 26, 2014, Federal Register (79 Fed. Reg. 58022). Comments may be mailed or faxed to the addresses and numbers listed in that notice.

Comments due by October 27, 2014

Comments are due by October 27, 2014, for the following forms and information gathering activities, which have already been submitted to the OMB for approval: Application for Search of Census Records for Proof of Age (SSA-1535-U3); Public Information Campaign; Medicare Subsidy Quality Review Forms (SSA-9301 through SSA-9304, SSA-9308, SSA-9310 through SSSA-9314, SSA-8510); Application to Collect a Fee for Payee Service; Redetermination of Eligibility for Help with Medicare Prescription Drug Plan Costs (SSA-1026-OCR-MS-SCE and SSA-1026-OCR-SM-REDE); Electronic Records Express (Third Parties)(ERE); and Screen Pop (an automated telephone process).

Comments due by November 25, 2014

Comments are due by November 25, 2014, for the following form, which has not yet been submitted to the OMB for approval: Request to Show Cause for Failure to Appear (HA-L90 (paper or Electronic Records Express)).
