The IRS has provided the general rules and specifications for reproducing paper and computer-generated paper substitutes for the January 2008 revision of Form 941, Employer's QUARTERLY Federal Tax Return, and for the January 2006 revision of Schedule B (Form 941), Report of Tax Liability for Semiweekly Schedule Depositors. Substitute territorial Forms 941-PR, 941-SS and Anexo B (Forma 941-PR) should also follow these specifications.
Changes to note

(2) The room number in the address of the Substitute Forms Unit has changed to 6526.

(4) The IRS has separated the entry space for the third-party designee's name and a telephone number in Part 4 on page 2. In addition, the IRS has moved the text for “Personal Identification Number” and the fill-in boxes for the number to the right to line up under “Designee's name and phone number.”
(5) The IRS has added “(or yours if self employed)” to the firm's name space in Part 6.
(6) The wording of the second line of text for Line 12 was changed from “Follow the Instructions for Form 941-V, Payment Voucher” to “For information on how to pay, see the instructions.”
(7) The IRS has added “to avoid a penalty” at the beginning of the Form 941-V instructions section “Making Payments With Form 941.”
(8) The wording in the “Caution” of Form 941-V instructions has been changed.
