SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding a number of forms and information gathering activities that have already been submitted for clearance by the Office of Management and Budget (OMB), as well as for some that are still pending at the SSA.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in notices in the July 2, 2014, and July 3, 2014, Federal Register (79 Fed. Reg. 37828 and 79 Fed. Reg. 38107). Comments may be mailed or faxed to the addresses and numbers listed in the notices.

Comments due September 2, 2014

Comments are due September 2, 2014, for the following forms and information gathering activities, which have not yet been submitted to the OMB for clearance: Medical Report on Adult with Allegation of Human Immunodeficiency Virus Infection, Medical Report on Child with Allegation of Human Immunodeficiency Virus Infection (SSA-4814-F5 and SSA-4815-F6); SSI Notice of Interim Assistance Reimbursement (IAR); Medical Source Statement of Ability To Do Work Related Activities (Physical and Mental)(HA-1151 and HA-1152); Application for Access to SSA Systems (SSA-120, paper and Internet versions); Statement of Funds You Provided to Another and Statement of Funds You Received (SSA-821-BK); Coverage of Employees of State and Local Governments; and Marital Relationship Questionnaire (SSA-4178).
