SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding several forms and an information gathering activity prior to their submission for clearance by the Office of Management and Budget (OMB), as well as for some forms for which clearance has already been sought.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in a notice in the March 28, 2014, Federal Register (79 Fed. Reg. 17632). Comments may be mailed or faxed to the addresses and numbers listed in that notice.

Comments due by May 27, 2014

Comments are due by May 27, 2014, for the following forms and information gathering activity, which have not yet been submitted to the OMB for approval: Representative Payee Report-Adult, Representative Payee Report-Child, Representative Payee Report-Organizational Representative Payees (SSA-623, SSA-6230, SSA-6234, and the electronic Internet application: Internet Representative Payee Accounting (iRPA)); Statement of Income and Resources (SSA-8010-BK); Authorization to Obtain Earnings Data From the Social Security Administration (SSA-581); State Death Match Collections (manual process and Electronic Death Registration Process (EDR)); and Generic Clearance for the Collection of Qualitative Feedback on Agency Service Delivery.

Comments due by April 28, 2014

Comments are due by April 28, 2014, for the following forms, which have already been submitted to the OMB for approval: Student Reporting Form (SSA-1383, SSA-1383-FC); Reporting Events-SSI (SSA-8150); Government Pension Questionnaire (SSA-3885); Modified Benefit Formula Questionnaire (SSA-150); and Employee Identification Statement (SSA-4156).
